I have acid reflux, and when my condition worsened, my doctor wanted me to take my medication twice a day, but my insurance company would not approve the increased dosage. The nurse at my doctor’s office made numerous phone calls, filled out all the necessary paperwork, and I was still denied. After the nurse redid my entire prior-authorization application, my insurer finally approved the medication, but only for a year.  During that time, the increased dosage of the medication helped tremendously – I very rarely had any heartburn. Once the year was up, the insurer denied the dosage increase again, and I had to pay out-of-pocket for the extra pills.
